When I try to connect my new MacBook 12" 2016 with a Dell P2715Q through the hdmi cable and the Apple HDMI/USB-C adapter it flickers or blacks out.
I tried different hdmi cables (that works), two HDMI/USB-C adapters, cleaning up the PRAM from the mac and resetting to factory settings on the monitor, but no improvement.
The model is a rev A03 from June 2015 with serial number: <Admin note = Private data deleted>

Unfortunately, this is not known by Dell. As you can see in the image below taken directly from the Dell monitor driver page,
the Apple proprietary operating system and hardware are not tested by Dell on this monitor. So this will be best guess support. You should also post on the Apple Discussions Forum and see if they can offer some insight or troubleshooting steps.
The adapter you have there is limiting you to =1920x1200 60Hz3840x2160 30Hz
So first verify that the Hz setting for the resolution is correct in the Apple OS. How, no idea.

On the Dell p2715q specs, it reads:
Native Resolution:
4K 3840 x 2160 ( DisplayPort: 60 Hz, HDMI: 30 Hz )
So I do not understand why the apple AV adapter cannot work as it operates with hdmi at UHD 30Hz as well...
Since we never tested the adapter nor the Apple hardware, we cannot know why. You need to ask the Apple users on their Forum.
